Approximately 70% of patients experience pain that is moderate, severe or extreme after Surgery, a problem which may be due to the use of <strong>Generalized Guidelines<\/strong> that do not consider the effect of varying analgesic efficacy between different operative procedures.<\/p>\n<p>As analgesic and technical advances are made in <strong>Acute Pain Management<\/strong>, it is increasingly important to <strong>Evaluate the Options<\/strong>, considering the <strong>Operative Procedure<\/strong> as well as <strong>Patient Risk Factors<\/strong> to determine the most favourable <strong>Regimen<\/strong> for <strong>Enhanced Patient Recovery<\/strong>.<\/p>\n<p><em><strong>The PROSPECT Initiative<\/strong><\/em><\/p>\n<p>The <strong>PROSPECT<\/strong> Initiative\u00a0(<a href=\"http:\/\/www.postoppain.org\/\" target=\"_blank\" rel=\"noopener\">www.postoppain.org<\/a>) presents <strong>Evidence &#8211; Based Guidelines for Postoperative Pain Management<\/strong>, formulated by <strong>Consensus Opinion<\/strong> of an <a href=\"http:\/\/www.postoppain.org\/working_group.aspx\" target=\"_blank\" rel=\"nofollow noopener noreferrer\">International Working Group<\/a>, a unique collaboration among Surgeons, as well as Anaesthesiologists.<\/p>\n<p><strong>Recommendations<\/strong> are based on Evidence from <strong>Procedure &#8211; Specific Systematic Reviews<\/strong>, and consider the <strong>Balance of Risks and Benefits<\/strong> for each intervention in the context of clinical practice. The <strong>PROSPECT<\/strong> Working Group formulates Recommendations for the use of analgesic, anaesthetic and surgical techniques in different operative procedures.<\/p>\n<p><strong><em>The PROSPECT Website<\/em><\/strong><\/p>\n<p>The <strong>PROSPECT<\/strong> website is used internationally, and provides clinicians with the <strong>Evidence<\/strong> to make <strong>Informed Decisions<\/strong> about <strong>Pain Therapy<\/strong>. Based on experience from <strong>PROSPECT<\/strong> Reviews, <strong>PROSPECT Methodology<\/strong> is continuously refined to take greater account of the quality of available evidence, to increase accuracy and reduce ambiguity, to reduce potential bias, and to enable the user to better judge the Strength of the Recommendations.<\/p>\n<p><em><strong>Acknowledgements<\/strong><\/em><\/p>\n<p>At this point, the Directory Board of ESRA Hellas would like to thank the PROSPECT Working Group and especially its <strong>President Professor Francis Bonnet<\/strong>, as well as <strong>Professor Narinder Rawal<\/strong>, for their unreserved help, their enthousiasm towards our efforts, their kindness, as well as their permission to use and upload scientific material and information from PROSPECT website in the ESRA Hellas web page.<\/p>\n<p>In this way, we believe that we will contribute first, to the promotion of ESRA Goals in Greece and second and more importantly, to the <strong>Provision and Spread of Knowledge<\/strong> to all <strong>Greek Anaesthesiologists<\/strong>, aiming in <strong>Improved Management of Surgical Patients<\/strong> and in <strong>Application of Evidence &#8211; Based Methods of Postoperative Analgesia<\/strong> in everyday clinical practice.<\/p>\n\t\t<\/div>\n\t<\/div>\n\t\n\t<div id=\"accordion-1215124329\" class=\"accordion-item\">\n\t\t<a id=\"accordion-1215124329-label\" class=\"accordion-title plain\" href=\"#accordion-item-prospect-work\" aria-expanded=\"false\" aria-controls=\"accordion-1215124329-content\">\n\t\t\t<button class=\"toggle\" aria-label=\"Toggle\"><i class=\"icon-angle-down\" aria-hidden=\"true\"><\/i><\/button>\n\t\t\t<span>PROSPECT Work<\/span>\n\t\t<\/a>\n\t\t<div id=\"accordion-1215124329-content\" class=\"accordion-inner\" aria-labelledby=\"accordion-1215124329-label\">\n\t\t\t\n<p><strong>PROSPECT <\/strong><a href=\"http:\/\/www.postoppain.org\/\">www.postoppain.org<\/a><\/p>\n<p>The <strong>Type<\/strong>, <strong>Level<\/strong> and <strong>Duration of Pain<\/strong> may vary depending on the <strong>Type of Surgery<\/strong>, e.g. thoracic versus abdominal or versus minimally invasive surgery.<\/p>\n<p>Some <strong>Analgesics<\/strong> are only relevant for <strong>Specific Operations<\/strong> (eg intraperitoneal local anaesthetics, peripheral nerve blocks).<\/p>\n<p>The <strong>Risks and Benefits<\/strong> of Different <strong>Analgesics<\/strong> differ between <strong>Procedures<\/strong> (eg general versus neuraxial anaesthesia).<\/p>\n<p><strong>PROSPECT<\/strong>, with its suggested perioperative directions, provides a new &#8220;<strong>Clinical Tool<\/strong>&#8221; for <strong>Postoperative Pain Management<\/strong> in common surgical procedures, by offering to clinicians <strong>Recommendations<\/strong> to support clinical decisions. These web &#8211; based Recommendations are quick, offering easy access to relevant information, in contrat to the <strong>Classic Guidelines<\/strong>, which are usually prescriptive, accompanied by linear documents, as well as lengthy and cumbersome to use.<\/p>\n<p><strong>PROSPECT Recommendations<\/strong> are based on <strong>Procedure &#8211; Specific Evidence from Systematic Reviews<\/strong> of the Literature, usually incorporating <strong>Randomized Controlled Trials<\/strong> of analgesic, anaesthetic and operative interventions affecting Postoperative Pain. It also provides <strong>Transferable Evidence<\/strong> from other Surgical Procedures, thus contributing to everyday Clinical Practice, through the practical guidance from the <strong>PROSPECT Working Group<\/strong>.<\/p>\n<p><strong>PROSPECT<\/strong> presents the <strong>Evidence<\/strong> as <strong>Summary Statements<\/strong>, with arguments for and against an analgesic, anaesthetic or operative technique, together with <strong>Links<\/strong> to the corresponding abstracts.<\/p>\n\t\t<\/div>\n\t<\/div>\n\t\n\t<div id=\"accordion-559693766\" class=\"accordion-item\">\n\t\t<a id=\"accordion-559693766-label\" class=\"accordion-title plain\" href=\"#accordion-item-prospect-methodology\" aria-expanded=\"false\" aria-controls=\"accordion-559693766-content\">\n\t\t\t<button class=\"toggle\" aria-label=\"Toggle\"><i class=\"icon-angle-down\" aria-hidden=\"true\"><\/i><\/button>\n\t\t\t<span>PROSPECT Methodology<\/span>\n\t\t<\/a>\n\t\t<div id=\"accordion-559693766-content\" class=\"accordion-inner\" aria-labelledby=\"accordion-559693766-label\">\n\t\t\t\n<p><strong>PROSPECT<\/strong>\u00a0\u00a0\u00a0\u00a0<a href=\"http:\/\/www.postoppain.org\/\">www.postoppain.org<\/a><\/p>\n<p><strong>Introduction<\/strong><\/p>\n<p>\u0397\u00a0<strong>PROSPECT <\/strong>provides recommendations for Procedure &#8211; Specific Postoperative Pain Management based on expert interpretation of Procedure &#8211; Specific and Transferable Evidence in the context of current clinical practice.<\/p>\n<p>The Process by which the <strong>PROSPECT<\/strong> Working Group formulates its Recommendations is continuously refined to take greater account of the Quality of available Evidence and to reduce the potential for bias in the formulation of Recommendations. The refined Methodology has been implemented in full for procedure reviews performed from 2006.<\/p>\n<p><strong>PROSPECT Methodology and Process<\/strong><\/p>\n<p>The <strong>PROSPECT Review Process<\/strong> begins with a <strong>Systematic Literature Review<\/strong>, based on the <strong>Protocol of the Cochrane Collaboration<\/strong>. The specific <strong>Inclusion Criteria<\/strong> for the review require <strong>Randomized Studies<\/strong> reporting <strong>Pain Scores<\/strong> on a Visual Analogue or Verbal \/ Numerical Rating Scale. Two or three <strong>PROSPECT Working Group<\/strong> Members (and usually an Expert Clinician external to the <strong>PROSPECT<\/strong> Group) with particular interest and expertise in the Operative Procedure to be reviewed, are closely involved in designing the <strong>Review Strategy<\/strong>, together with a <strong>Medical Writing Team<\/strong>. Where there is limited <strong>Procedure &#8211; Specific Evidence<\/strong> regarding the Analgesic Effects of a particular intervention, the Group may agree to include <strong>Supplementary \u201cTransferable\u201d Evidence<\/strong> from other procedures with a comparable pain profile. Transferable Evidence regarding adverse effects may be extracted from a variety of other surgical procedures. Members of the <strong>PROSPECT Working Group<\/strong> first comment on the review using the <strong>Delphi Method<\/strong>, and thereafter collated comments are shared and discussed until a <strong>Group Consensus<\/strong> is achieved.<\/p>\n<p><strong>PROSPECT Working Group<\/strong><\/p>\n<ul>\n<li>The <strong>Working Group<\/strong> is an <strong>International Collaboration of Surgeons and Anaesthesiologists<\/strong>, chaired by Professor Francis Bonnet<\/li>\n<\/ul>\n<ul>\n<li>The <strong>Working Group<\/strong> reviews in detail the evidence from the <strong>Procedure &#8211; Specific Systematic Review<\/strong>, as well as the <strong>Transferable Evidence<\/strong>, the <strong>Clinical Practice Information<\/strong> and the <strong>Draft Recommendations agreed<\/strong> at the Subgroup meeting. For each analgesic, anaesthetic or surgical intervention, the group first comes to a consensus about the <strong>Applicability<\/strong> and <strong>Relevance<\/strong> of transferable evidence and clinical practice information. The group then refines each Recommendation according to Consensus, considering all of the evidence and information.<\/li>\n<\/ul>\n<ul>\n<li>Simultaneously, apart from the Working Group, there are
